Title: Incidents Reporting System and Mobile Application

Abstract: Incident Report Systems are used to gather information from users and report incidents to the responsible institutes or owners. This would happen from filling forms, verbal complaints and digital submissions to the relative party. The main objective in reporting an incident is to prevent similar occurrences in the future and identify reasons for the incident. Any unplanned event or near miss occurrences are considered as an incident and the responsible person or organization is called as incident owner. The aim of this project is develop a web based incident management system and Android application to report incidents or occurrences immediately to relevant organizations. The system is connected to MySQL database with an Android application. Get data and post data methods are used for communicating with database and Android application. The Android application is posting data to the web application and receives data posting accordingly to the database tables. The dashboard is indicating all incidents which are categorized by incident types to identify. This system has been designed according to the standard web based model. The system was developed as a set of components to make it easy to design, understand and implement. The system has been developed using PHP, MySQL, Apache and Android development kit. The system has been implemented to work in the Microsoft Windows operating system environment. Testing codes had been used to validate system correctness and system functionalities according to the plan. The obtained results are clearly indicating the success of the project and its usefulness when it comes to real users.
