Title: Patient Information Management System for District Hospital Polpithigama

Abstract: Considerably District Hospital, Polpithigama is a very busy place with more than 500 patients, medical staff, administrative staff and labors. It is one of the best district hospitals in the North Western Province. Handling records of a vast number of patients manually is almost an impossible task when we consider the time and energy consumed and also it is difficult to access some important information through the manual books. Hospital is managed with a manual file system. As a result, the administration is facing several problems like data inaccuracy, data redundancy and impediment in decision making due to unavailability of timely reports and information of patients. Mainly the project is focused on building a common web based Patient Information Management System to overcome some of the problems and inefficiencies of the current manual system. System input contains patient details, diagnosis details, clinical details of patients while system output is to get these details on to the reports by developing modules such as: patient registration, patient history management, clinical and OPD patient management, in-patient management, login management and report generation Further, the most important part is; the web based system makes it possible to access the patient treatment details from any location. The system has adapted according to the object oriented techniques. In addition, Unified Modeling Language (UML) is used for analysis and designing. Hypertext Preprocessor (PHP) which is a server-side scripting language is used to build the system. Meanwhile, MySQL is used to handle databases and Apache has been used as the web server. Proper achievement of the goals and objectives of a hospital heavily depends on better patient care and patient information controlling mechanism. After passing through the stage of implementation of the results obtained, data security is more ensured because it is equipped with user validation process and stores data methodically in report preparation so the time consumption is shorter than ever before. Hence, I believe that the project work will create a better and an efficient working environment in District Hospital, Polpithigama.
